The problem with fragmented ADHD care

For many people seeking an ADHD assessment, the process does not end with diagnosis—it becomes more complicated.

It is not uncommon for individuals to receive a diagnosis, only to then face uncertainty about what happens next. Medication pathways can be unclear, waiting times can be long, and navigating between different providers can feel disjointed. This can leave people in a difficult position: having clarity about their difficulties, but limited support in moving forward.

In my view, this gap between assessment and treatment is one of the most significant challenges in current ADHD care.


A more integrated way of working

In response to this, I have developed a more joined-up approach.

Alongside providing comprehensive, NICE-informed ADHD assessments, I now collaborate with an experienced and independent nurse prescriber. This allows me to offer clients a clearer and more streamlined pathway from assessment through to treatment, where this is appropriate.

The aim is not to change the integrity of the assessment process, but to ensure that once an outcome is reached, there is a clear and timely next step available.


How the pathway works

The process begins with a full ADHD assessment, where I take a detailed developmental history and explore current functioning across different areas of life. This includes clinical interviews, structured assessment tools, and careful consideration of alternative explanations.

Following the assessment, I provide a clear outcome. In some cases, this may confirm ADHD; in others, it may not. Where ADHD is identified, we discuss a range of treatment options, including both psychological and non-pharmacological approaches.

For those who wish to explore medication, I can now offer a direct referral to a nurse prescriber with whom I work closely. This enables clients to access a medication appointment promptly, without needing to independently search for a suitable provider or navigate complex systems.


Clear roles and clinical responsibility

It is important to be clear about how this collaboration works.

I remain fully responsible for the assessment, diagnosis, and clinical recommendations. The nurse prescriber works independently, with their own professional registration and governance, and is solely responsible for any prescribing decisions and medication management.

This ensures that both assessment and treatment are delivered within appropriate professional boundaries, while still offering a coordinated experience for the client.


What this means for clients

A more integrated pathway offers a number of practical benefits. It reduces uncertainty about next steps, shortens the time between diagnosis and potential treatment, and removes the need to navigate multiple services independently.

Just as importantly, it provides a sense of continuity. Rather than feeling that the assessment is a standalone event, clients are supported through a more cohesive process that considers what happens next.


A balanced and considered approach

Not everyone who is assessed for ADHD will receive a diagnosis, and not everyone who is diagnosed will require or choose medication. These decisions are always made carefully, based on individual need, clinical judgement, and informed discussion.

This pathway is designed to offer clarity and access—not to rush or pre-determine outcomes.
